6eb177091b overhaul of MotionState cleanup
Moved MotionState deletes out of PhysicsEngine.
EntityMotionStates are deleted by the PhysicsEntitySimulation.
AvatarMotionStates are deleted in the Avatar dtor.

cf74dbe1dc partial progress toward sim ownership negotiations
